Service Description
Here Alpine Health, we we look at each of the 6 pillars of lifestyle medicine and teach healthy habits alongside concrete coping skills, to help you break the cycle of feeling bad and get on track with healthy habits, so you can boost your energy and feel amazing! In this private session with one of our doctors, we'll go over all the things that you wish you were doing, help confront what's dragging you down, and create a personalized action plan to move you along the path to your health goals. The session includes: 1. Lifestyle Health Assessment: we'll evaluate your sleep, nutrition, exercise, stress, social support, and substance use (this includes anything you use to numb - smoking, drinking, even binge watching TV) 2. We'll use that snapshot of your overall health and wellness to help you prioritize your goals 3. Learn how to set attainable goals so we can guarantee a win! 4. You'll take home a SMART Action Plan that you can start on IMMEDIATELY. Whether you're a new member and you want a taste of what we do, or you want to check in and move on to your next goal, we'll use the time in this session to keep you on track and get you feeling great! When you schedule your session, please indicate in the comments whether you'd like an overview goal setting session, or if there's a specific health goal you're already hoping to tackle.
